Message Media Scripts
Written scripts with consistent verbiage are used to ensure PII is not included in any written messages or recordings left for DEBS clients. Messages and recordings are typically communicated by:
- Telephone (cellular or land line),
- Text, and/or
- E-Mail.
Reminder: When communicating via email or text message, there must be a signed “Consent to Receive Electronic Communications from the County of Santa Clara” (SCD 2551) on file allowing DEBS staff to send emails and /or text messages directly to the client.
Scripts
The following are scripts for eligibility staff to use when communicating with clients.
Add A Person Appointment
This is a message from Santa Clara County Social Services Agency. You have an appointment to add someone to your case. A valid ID, SSN and birth verification must be provided for this person. An interview appointment is scheduled for (MM/DD/YY) at (00:00). Please arrive on time at (office address here). If you are not able to keep your appointment please contact your worker at (phone #). If you do not complete this interview, your benefits will discontinue effective (MM/YY).
Note: This script may be revised to add/delete the necessary verifications and/or to change the interview to a telephone interview.
DMV Voucher Reminder
This is a message from Santa Clara County Social Services Agency. Your DMV Voucher will be ready for pick up at (00:00). Please arrive on time at (office address here). Please bring ID. If you are not able to keep your appointment please contact your worker at (phone #) to reschedule this appointment.
EBT Reminders
This is a message from Santa Clara County Social Services Agency. Your EBT card will be ready for pick up at (00:00) on (MM/DD). Please arrive on time at (office address here). Please bring ID. If you are not able to keep this appointment please contact your worker at (phone #) to schedule another appointment.
Missed Intake Appointment
This is a message from Santa Clara County Social Services Agency. You have missed your application appointment scheduled for (00:00) on (MM/DD/YY). Please contact your worker at (phone #) to schedule another appointment.
Missed RRR Appointment Reminder
This is a courtesy reminder from Santa Clara County Social Services Agency. You have missed your annual Telephonic Interview appointment which is due in (MM/YY). If you do not complete this interview, your benefits will discontinue effective (MM/YY). Please contact your worker at (phone #) to schedule another appointment.
SAR7 Reminder
This is a courtesy reminder from Santa Clara County Social Services Agency. We have not received your report for (MM/YY). If a report is not submitted by (MM/DD/YY) your case will be discontinued effective (MM/YY). You can submit your completed report at www.mybenefitscalwin.org, or mail/bring it to your nearest SSA office. You can contact your worker at (phone #).
