Recertification Forms

The forms below must be used to complete a CalFresh Recertification.
The series of application forms that are used are based on:
- The type of CalFresh household
- Whether the interview is to be conducted face-to-face or over the telephone
Primary Application Forms
CalFresh only cases:
- Recertification for CalFresh Benefits (CF 37)
CalFresh and CalWORKs or GA or Medi-Cal combo cases:
- Application for CalFresh, Cash Aid, and/or Medi-Cal/Health Care Programs (SAWS 2 Plus), and
- Rights and Responsibilities and Other Important Information (SAWS 2A SAR)

Note: When recertification is processed by paper application, benefits must not be interrupted if forms are returned, except for the application for Recertification (i.e., CF 37).
If a valid application is on file at intake or redetermination (RD) for a CalWORKs/CalFresh combo case, and the SAWS 2 Plus/Statement of Facts (SOF) is signed to meet CalWORKs requirements (such as due to incomplete or incorrect information on the application being corrected), the presence of an additional signature in a combo case is not considered a duplicate signature for CalFresh. Although CalFresh is a single‑signature program, an additional signature collected in a combo case is treated solely as a CalWORKs requirement. The reason for the additional signature must be clearly documented that the second signature is required only for CalWORKs, not for CalFresh.
