Policy and Report of Electronic Theft of Benefits (EBT 2259)

Background
Previously EBT cardholders have been required to file a police report with their local Law Enforcement Agency (LEA), as well as file a theft claim with the EBT vendor (via the EBT Customer Service Helpline) to complete the electronic theft claim process for cash benefits. If only CalFresh benefits had been stolen, EBT cardholders were not required to file a police report.
Changes
The State has updated policy and revised the form, Report of Electronic Theft of Benefits (EBT 2259), to remove barriers that cardholders may face, as well as updated the instructions for completing the form.
The revised policy and EBT 2259 eliminates the following cardholder requirements:
- Filing a claim with the EBT vendor.
- Filing a police report with their local LEA.
The Electronic Theft section in Common Place HB chapter 16 Electronic Benefit Transfer (EBT), has been revised. Please note that many of the subsections within the Electronic Theft section have also been reformatted.
Implementation
Effective immediately.
