Change of Address (SCD 2339)

When the Kin-GAP guardians have a change of address that does not come from the DFCS SW, the FC EW will provide the legal guardian with “Relative Guardians Address Verification” form (SCD 2339) before changing the address. When there is more than one legal guardian each person must sign and date the SCD 2339 and return it to FCE. After the SCD 2339 is received and the address has been updated, the SCD 2339 is forwarded to Imaging.

  • Note: When the SCD 2339 is not received and the FC EW is aware that benefits will be issued to an incorrect address, benefits may be placed on hold until the SCD 2339 is received.

Exception: The Kin-GAP change of address is added only to federally eligible Kin-GAP cases initiated through the ICAMA process [Refer to Interstate Compact on Adoption and Medical Assistance (ICAMA).

Once the SCD 2339 has been received, then the FC EW will enter the address change in CalSAWS and review MEDS the following day to ensure the new county of residence appears, if applicable.

Optional Form

The “Kin-GAP Eligibility Checklist” (SCD 23) is optional but was developed to assist in gathering necessary documents during application and to serve as a reminder for other necessary actions in CalSAWS and MEDS.

Child Support Referrals

Effective January 1, 2023, Assembly Bill (AB) 1686 directs counties to presume that the payment of child support by the parent is likely to pose a barrier to the proposed reunification. This rule extends to Kin-GAP cases. Therefore, counties shall no longer refer parents to LCSA unless the case meets exception criteria. For more information refer to Child/Medical Support Overview.

Social Security Number (SSN)

A social security number is required for all children in the Kin-GAP Program.
