Change of Address Within California

A request for an address change can be initiated by the PASW or adoptive parent. The PASW can communicate the address change by sending the forms “Adoptive Parent Address Verification” form (SCD 2339A) or "Payment Instructions Adoption Assistance Program" form (AAP 2) to the EW.

Note: The AAP 3 form cannot be used to verify an address change.


When the adoptive parents have a change of address that does not come from the Adoption SW, the AAP EW will provide the adoptive parent with the SCD 2339A before changing the address. If there are multiple adoptive parents, both parents must sign and date the SCD 2339A and return it to FCE. The SCD 2339A must be legible and can be received via mail, email (scanned-no pictures), or electronically signed via DocuSign. Upon receipt of the SCD 2339A, the AAP EW will update the address in CalSAWS, create a Journal entry, forward the document to Imaging, and verify MEDS for address update. Once the SCD 2339A has been processed, the forms should be emailed to the PASW.

Note: When the SCD 2339A is not received and the AAP EW is aware that benefits will be issued to an incorrect address, benefits may be placed on hold until the SCD 2339A is received.
