Application for Non-Dependent Legal Guardians

The workflow below is used for non-dependent legal guardianships which were established in either Probate or Juvenile court.

  1. Legal Guardian
    1. Calls or comes to FCE to request Foster Care.
  2. FCE Clerical
    1. Receives the referral from the guardian and confirms that:
      1. The LG is not a relative,
      2. There is no court dependency, and
      3. The LG has a temporary or permanent letter of guardianship.
    2. Collects information needed to complete the “Identification and Intake Record” form (SCD 41) and the “Initial Application for CalFresh, Cash Aid and/or Medi-Cal/Health Care programs” form (SAWS 2 Plus).
    3. Completes application registration for FC and Medi-Cal.
    4. Sends intake packet to the LG with instructions.
    5. Assigns case to Foster Care Intake.
  3. Intake EW
    1. Calls the LG to ask if they would like an in-person intake appointment.
    2. Note, face to face appointments are not necessary.

    3. If yes, then advises LG to bring intake packet documents and the letter of guardianship.
    4. After the appointment, forwards the following documents to DFCS clerical: A completed “Child Welfare Intake for Non- Dependent Children with Guardians” form (SCD 1489), Letter of Guardianship, and child’s birth certificate.
  4. SW
    1. Receives documents listed above from the Intake EW.
    2. Interviews LG, completes an assessment and service plan.
    3. Generates SOC 158A “Foster Child’s Data Record and AFDC-FC Certification” form (SOC 158A) which is sent to FCE.
  5. Intake EW
    1. Receives SOC 158A and authorizes initial payment.
      1. Note: Payment is issued from the date the SAWS 2 Plus was signed or the date of the legal guardianship letter, whichever is later.
