Incoming Inter-County Transfer (ICT)

  1. Sending County/DFCS
    1. Sends/Generates ICT packet.
  2. FC Clerical
    1. Receives ICT packet or SOC 158A.
    2. Completes ID.
    3. Forwards documents to Triage.
  3. Triage
    1. Reviews documents for complete ICT from sending county.
    2. Completes the appropriate Checklist Page 2 Step 3.
    3. SOC 158A only or incomplete packet
      1. If SOC 158A only/ incomplete ICT, then
        1. Contacts sending county to request ICT packet.
        2. Contacts SW in our county for the missing documents.
      2. If Court Dependency, then 
        1. Searches CWS/CMS for transfer of jurisdiction to Santa Clara County.
        2. Obtains copy of Minute Order.
        3. Contacts SW for FC 2 and SOC 158A.
    4. Follows escalation process if documents are not received.
    5. Forwards documents and form SCD 2321 to designated FC Clerical for app. reg. and case assignment. Triage must write “ICT” written on upper right hand corner of the SCD 2321.
  4. Designated FC Clerical
    1. Verifies the correct mailing address when provider exists.
    2. Adds the provider only if a FH placement (except SILP) the first day of the month when provider does not exist.
    3. Completes Application Registration selecting “ICT” as the application type in CalSAWS.
    4. Enters CalSAWS case number in CWS/CMS.
    5. Assigns case.
    6. Forwards documents to assigned Intake EW.
  5. Intake EW
    1. Receives case assignment.
    2. Reviews the ICT packet for required forms/documentation.
      1. If the packet is incomplete (missed by triage), then returns to triage.
      2. If after triage review, new information is received, then contacts sending county EW/DFCS SW as appropriate for missing information.
    3. Follows Intake Benefits Processing
