Incoming Inter-County Transfer (ICT)
- Sending County/DFCS
- Sends/Generates ICT packet.
- FC Clerical
- Receives ICT packet or SOC 158A.
- Completes ID.
- Forwards documents to Triage.
- Triage
- Reviews documents for complete ICT from sending county.
- Completes the appropriate Checklist Page 2 Step 3.
- SOC 158A only or incomplete packet
- If SOC 158A only/ incomplete ICT, then
- Contacts sending county to request ICT packet.
- Contacts SW in our county for the missing documents.
- If Court Dependency, then
- Searches CWS/CMS for transfer of jurisdiction to Santa Clara County.
- Obtains copy of Minute Order.
- Contacts SW for FC 2 and SOC 158A.
- If SOC 158A only/ incomplete ICT, then
- Follows escalation process if documents are not received.
- Forwards documents and form SCD 2321 to designated FC Clerical for app. reg. and case assignment. Triage must write “ICT” written on upper right hand corner of the SCD 2321.
- Designated FC Clerical
- Verifies the correct mailing address when provider exists.
- Adds the provider only if a FH placement (except SILP) the first day of the month when provider does not exist.
- Completes Application Registration selecting “ICT” as the application type in CalSAWS.
- Enters CalSAWS case number in CWS/CMS.
- Assigns case.
- Forwards documents to assigned Intake EW.
- Intake EW
- Receives case assignment.
- Reviews the ICT packet for required forms/documentation.
- If the packet is incomplete (missed by triage), then returns to triage.
- If after triage review, new information is received, then contacts sending county EW/DFCS SW as appropriate for missing information.
- Follows Intake Benefits Processing
