Receiving Assessment Intake Center (RAIC) or Placement Tracking Team (PTT) Application (Most New Admits)

Medi-Cal Applications For RAIC (includes children/youths with and without FC placements.

  1. RAIC
    1. When a child is brought to the RAIC, the staff:
    2. Admits the child to RAIC.
    3. Completes the RAIC admit log sheet, bed list, SAWS 2 Plus, SOC 158A, “Emergency Shelter Housing” (ESH) form and if necessary, the “Presumptive Transfer Request” form.
    4. Identifies if the child is on MEDS and CWS/CMS.
    5. Sends an admit packet to FCE clerical and DFCS ER clerical either by e-mail or by fax.
  2. DFCS
    1. Processes SOC 158A.
    2. Forwards information to FC Clerical.
  3. Designated FC Clerical
    1. Receives the admit packet from DFCS/RAIC either by e-mail or by fax.
    2. Completes the child ID process in CalSAWS, MEDS and CWS/CMS.
    3. Completes an SCD 41 when other CalSAWS cases are open outside of FCE.
      1. If the child is active in MEDS with no share of cost (SOC), then prints a temporary paper BIC and attaches to the intake packet.
      2. If the child is active in MEDS with a SOC, then 
        1. Performs application registration to create a new Medi-Cal case number using the child’s existing CIN.
        2. Prints a temporary paper BIC and attaches to the intake packet.
      3. If the child is active in MEDS with OHC, then notes OHC on the SCD 41.
      4. If the child is not in MEDS, then
        1. Completes minimal information on the “Application for Cash Aid, Food Stamps, and/or Medical Assistance” (SAWS 2 Plus), using RAIC address.
        2. Completes app. reg.
        3. Forwards documents to Triage.
  4. Designated FC Clerical
    1. IF A PRESUMPTIVE TRANSFER REQUEST HAS BEEN RECEIVED...
      1. Designated FC Clerical
        1. Completes minimal information on the “Application for Cash Aid, Food Stamps, and/or Medical Assistance” (SAWS 2 Plus), using RAIC address.
        2. Completes app. reg.
        3. Creates “urgent” task for Presumptive Transfer request for EW.
        4. Walks task to EW. If EW is not available, then walks task to SSPM for urgent assignment.
      2. EW
        1. Completes “Request for Online Transaction” form (SCD 1296) MTO for EW15 Online Transaction-Immediate Need to ensure residence address, residence county code, and aid code 45 for Medi-Cal will be reflected in MEDS.
        2. Verifies the following day that the MEDS entries are correct.
        3. Enters journal indicating that case is pending verifications and that Presumptive Transfer request has been completed.
        4. Sends email to requestor, assigned SW/PO and SW/PO Supervisor to confirm Presumptive Transfer has been completed. 
    2. Designated FC Clerical
      1. Adds a copy of MEDS Q1 screen to the Intake packet.
      2. Forwards the Intake packet to the Triage EW.
      3. Assigns the new Medi-Cal case to the Intake control caseload CFCP or the appropriate case number.
  5. Triage
    1. Follows the Triage Process.
  6. Designated FC Clerical
    1. Performs case assignment function in CalSAWS.
    2. Forwards received intake packet from Triage EW to Intake EW.
  7. Intake EW
    1. MEDI-CAL ONLY
      1. Receives and reviews RAIC sheet.
      2. Completes the MC 250 for DFCS children.
      3. Sets up the Medi-Cal case according to established process.
      4. Forwards copy of temporary BIC to assigned SW.
      5. Completes Intake Benefit Processing.
    2. FOSTER CARE PLACEMENT
      1. Receives intake packet and case working folder.
      2. Reviews CWS/CMS, CalSAWS and MEDS.
        1. Determines need for assistance.
        2. Determines the authority of placement.
        3. Completes Checklist when applicable.
        4. Requests all necessary documents from SW. 
      3. Completes Intake Benefits Processing.
      4. Forwards intake working folder with Checklist and attached SCD 163 form on top to EWS for review.
