Voluntary Placement Request Received by Bill Wilson Center (BWC) and DI

  1. BWC/DFCS/DI
    1. BWC faxes application SOC 155 and SCD 41 FCE.
  2. FC Clerical
    1. Receives fax from BWC.
    2. Completes the child ID process in CalSAWS, MEDS and CWS/CMS.
    3. Completes an SCD 41 when other CalSAWS cases are open outside of FCE. 
    4. Contacts Emergency Response (ER) screening unit within 24 hours.
    5. Adds a copy of MEDS Q1 screen to the intake packet.
    6. Forwards the intake packet to the Triage EW.
  3. Triage EW
    1. Follows the Triage Process.
  4. Designated FC Clerical
    1. Performs case assignment function in CalSAWS.
    2. Forwards all received intake packet from the Triage EW to Intake EW.
  5. FC Intake EW
    1. MEDI-CAL ONLY
      1. Receives and reviews RAIC sheet.
      2. Reviews MC 250 (for JPD children).
      3. Reviews SDC 692 (Title IV-E eligibility form).
      4. Completes the MC 250 for DFCS children.
      5. Sets up the Medi-Cal case according to established process.
      6. Forwards copy of temporary BIC to assigned SW.
      7. Follows Intake Benefits Processing.
    2. VOLUNTARY PLACEMENT PROCESSING
      1. Receives the documents and schedules an appointment.
      2. Notifies the SW if appointment needs to be rescheduled.
      3. Notifies FC Clerical of the appointment date and time.
        1. Note: A meeting with the parents normally occurs before meeting with the SW.

      4. Gathers necessary forms:
        1. EA 1,
        2. SOC 155,
        3. SOC 158A,
        4. FC 2,
        5. SAWS 1, and
        6. SCZ 414Z for payment prior to signing SOC 155.
      5. Determines length of placement.
      6. Completes Benefits Intake when SOC 158A and SCZ 414Z is received.
      7. Asks parents if they want child's Medi-Cal to continue when placement has ended.
        1. If YES, CalSAWS initiates CEC.
        2. If NO, discontinues and closes the case following the FC Case Closing Policy.
    3. PARENT/CARETAKER IS A NO-SHOW
      1. Contacts BWC and SW to confirm the status of the placement (the case can be processed without parents signature, but the SW/PO must complete the SOC 155, and case must be non-federal).
      2. Reschedules appointment, when necessary.
      3. Meets with parents.
      4. Authorizes denial for No Shows after no response from parents or SW.
      5. Verifies denial was issued and forwards a copy to assigned SW.
      6. Completes Benefits Intake.
