Director's Exception
A Department of Employment and Benefits Services (DEBS) Director’s Exception (DE) is needed to approve the issuance of the burial expense payment when the child is ineligible, and other placement expenses that are not covered by AFDC-FC for a Juvenile Probation Department (JPD) case.
Forms
The EW must complete either the:
- Foster Care Director’s Exception – Burial Expense form (FC 5)
- Foster Care Director’s Exception – Placement Expense form (FC 5 )
Referral Process
The Probation Officer (PO) must send the assigned EW an email requesting the expense and provide the following information:
- PO’s name, worker number (if any), office phone number
- Youth’s complete name, DOB, date of death (DOD) (for burial expense)
- The vendor/payee information: name, address, city, zip, phone, email address. If this is a business, include a contact name.
- List of the item(s) being requested. Include any payment(s) already made and the amount being requested. The EW should check the SCD 2405 for current rates.
- An itemized invoice, receipts, and other applicable documents.
- An explanation of why the request is needed, what other resources have been identified or exhausted, and the impact on the youth.
- Any other details needed for the completion of the Director’s Exemption form (FC 5).
After the information mentioned above has been provided:
The EW
- Reviews the request and documentation to ensure that all necessary information has been provided.
- Follows DocuSign instructions for Foster Care Director’s Exception.
- Completes the appropriate FC 5 (for burial or placement) and uploads all documentation in the DocuSign envelope.
- Sends a copy of all forms and documentation to Imaging.
The DocuSign template will be sent to the EW Supervisor, Foster Care SSPM, Foster Care Coordinator, DEBS Director, and the SSA Director for signature.
The Foster Care Coordinator
- Will receive a copy of the final DocuSign envelope and forward it to EW.
- Save all documents in the S drive folder for the Foster Care Director’s Exception.
