This alert is generated when both Form 1095-B and a Notice for Requested Action (NFRA) mailings are returned as undeliverable.

Action Required

Complete an ex parte review including verifications in imaging , companion cases (CalFresh/GA/CalWORKS), CalSAWS/CalHEERS and MEDS. If the EW locates the most recent address, update the new address in CalSAWS/CalHEERS and check MEDS in two working days. After the correction is made, a new 1095-B form with correct mailing address will be mailed to a client via batch process.
Follow the returned mail process when the EW is unable to locate the new address.
