IDENTITY-DOC-NUMBER
Identity Document Number. This data element identifies the unique document number of the document that was used as evidence of identity.
For driver's licenses and state identification cards, this field includes the two character code for the state or territory, followed by DL for driver's license or ID for identification card, followed by the unique client specific number on the document (e.g. CADLR1234567).
For all other documents, this field should include the unique client specific number on the document. If there is no unique client specific number on the document, leave the Doc # field on the Individual Demographics Detail page blank. For online entries completed by MTO, enter 'NONE' on the AP19 screen.
An identity document number is not required when the identity document type indicates a school record, written affidavit or Medi-Cal application (begins with a 7), an exempt status (begins with an 8), or the applicant is unable to obtain the required documentation (begins with a 9). When any other identity document type is reported, an identity document number must also be reported on the transaction.
