Background

Medi-Cal Managed Care Health Plans are health care organizations that contract with the California Department of Health Services to provide health care services to qualifying Medi-Cal recipients. Health Plans that provide services in multiple counties are assigned a different HCP-NUM for each county.

Changes

New HCP listed below is being activated in Santa Clara County as a result of the agreement between Department of Health Care Services (DHCS) and Managed Care Plans (MCP) to transform Medi-Cal into a more equitable health system that will result in better health outcomes for Californians.

• HCP#374 Kaiser Foundation Health Plan, Inc’ Santa Clara

Implementation

The change in this Update is effective January 1, 2024.

Handbooks

MEDS Handbook Chapter 12, Section HCP-NUM, has been updated to reflect the change.
