Fresh Success Work Flow

EWs are to refer interested, qualified CF students to the participating Fresh Success community college by following the Fresh Success Work Flow outlined below.

  1. EW
    1. Upon approving the CF application or discovering that a CF recipient is enrolled at a participating Fresh Success community college:
      1. Reviews to ensure the student is receiving Federal CF benefits and is not CWES sanctioned or on an active CalWORKs (CW) case.
        1. If the student is receiving State funded CF, CWES sanctioned or on active CW case, stop here. Do not refer the student to the Fresh Success Program and complete a Journal entry in CalSAWS.
        2. Asks the student if they would like to participate in a CFET Program through the participating community college.
          1. If student declines, then Stop here and enter complete a Journal entry in CalSAWS.
          2. If student accepts, then
            1. Provide the student with the informational flyer, “Need Help with College Expenses”, (SCD 2443).
            2. Ask the student if he/she consents to Santa Clara County sending the referral to the participating community college.
            3. If student does not consent then the referral cannot be sent.
    2. Completes a Referral to appropriate Fresh Success community college through the DEBS Referral Application. See the DEBS Referral Application User Guide and Program Directive 23-03: DEBS Referral Application for reference.
  2. Community College Fresh Success Staff
    1. Receives the referral and determines if student is eligible for the Fresh Success Program.
    2. Enrolls the student in the Fresh Success Program and assists the student to send the Pre-Enrollment Form (SCD 2445) and the Consent Form (SCD 2445A) or the Community College’s Consent Form via the DEBS Online Portal or student’s BenefitsCal account.
    3. If the student is eligible, sends a secure/encrypted email, including name and date of birth to the following email address based on the community college to inform County of determinationdetermination.

  3. Clerical
    1. Checks the e-mail inbox daily.
    2. Identifies the EW of record.
    3. Creates the TMT.
      1. Assigns the TMT to an EW the same day.
      2. Selects the TMT Priority as “Urgent”.
      3. Enters “Fresh Success” in the TMT Notes section.
  4. EW
    1. Receives the Urgent TMT and:
    2. Updates the E&T and Student information in CalSAWS within 3 days.
      1. Note: The SCD 2445 will indicate if the student is attending school at least half time.

Participating community colleges that offer the Fresh Success Program will refer students to Santa Clara County Social Services by following the Fresh Success Work Flow outlined below.

  1. Community College Fresh Success Staff
    1. Identifies if the student is receiving CF benefits.
      1. If the student is a CF benefits recipient:
        1. Pre-enrolls the student into the program.
        2. Upload the SCD 2445 and SCD 2445A or Community College Consent Form to either the DEBS Online Portal or the student’s BenefitsCal account.
    2. Sends a secure/encrypted email, including name and date of birth to the following email address following email address based on the community college to inform County of uploaded documents.

    3. If the student is not a CF benefits recipient:
      1. Assists the student to apply online via BenefitsCal.
      2. Ensures that both the SCD 2445 and SCD 2445A or Community College Consent Form are uploaded with the BCW application.
      3. Sends a secure/encrypted, including name and date of birth to one of the designated County email addresses above (based on the community college).
  1. Clerical
    1. Checks the designated e-mail inbox daily.
    2. Identifies the EW of record or assigns case.
      1. Creates a TMT and assigns it to an EW the same day.
      2. Sets the TMT Priority as “Urgent”.
      3. Enters “Fresh Success” in the TMT Notes section.
    3. New BCW applications are to be expedited, once registered:
      1. Creates an Intake TMT.
      2. Sets the Priority as “Urgent.”
      3. Enters “Fresh Success” in the TMT Notes section.
  2. EW
    1. Receives the “Urgent” TMT to process the BCW application or to process the SCD 2445 E&T request.
      1. Note: Requests must be processed within 3 business days.

    2. Reviews CalSAWS to ensure the student is receiving Federal CF benefits and is not CWES sanctioned on a active CW case.
      1. If the student is receiving State funded CF or CWES sanctioned on active CW case, stop here. Send a secure/encrypted email to freshsuccess@foundationccc.org, include student name, case number and reason E&T request was not completed and enter case comments.
    3. Complete the appropriate Student and E&T windows in CalSAWS.
