Foster Care Applications from the Children’s Shelter or PTT

Once a Foster Care application comes in from the shelter or the Placement Tracking Team (PTT, the PAC EW evaluates each child’s case, searches for as much information as possible and gathers the necessary documents to determine eligibility, including, but not limited to the following:

  • Searches CWS/CMS for history and court orders, Petition, and other needed information.
  • Contacts the Social Worker and requests required forms and documents, such as the FC 2, SOC 158A, Initial Petition, Minute Orders, Warrant Petition, SOC 155 and Home Approval documents, etc.
  • Searches CalSAWS and MEDS history to determine if the child is active in other cases/programs, then contacts the EW to discontinue their case if appropriate; and, requests copies of forms and documents from the other case as needed, if not available in imaging.
  • Completes the “PAC Checklist” and a case summary on “Contact-Action Summary” (SCD 163).
  • Forwards to PAC CST for Application Registration.
