Juvenile Probation Department (JPD)

Applications for Ward Children (under W & I Code Section 602) are initiated by the JPD. When a minor is placed in the appropriate facility, the Probation Officer completes a face sheet with the family history,

The Probation Officer prepares in-house documentation and is responsible for completing the Foster Care application forms and forwarding them to the PAC CST.

If the application is for Foster Care and Medi-Cal, then the form packet includes:

  • SAWS 2 Plus
  • FC 2
  • SOC 158A
  • Petition and Detention Orders
  • Placement Orders

If the application is for Medi-Cal only, then the form packet includes: 

  • SAWS 2 Plus
  • MC 250
  • Placement Orders
  • MC 13 (for a child who is not a US citizen only)

PAC CST

  • Picks up the applications weeky at JPD and stamps “date-in” on the JPD “face sheet” and makes a copy.
    • “Starts” (applications for new placements or Medi-Cal only)
    • “Stops” (ending placements), and
    • “Reinvestigations” (RE's).
  • Performs person search in CalSAWS to see if the child has a case, and writes the case number on the packet if there is one.
  • Completes Application Registration.
  • Forwards the forms packet and face sheet to the PAC EW III.

PAC EW III

  • Reviews the application forms and may need to ID child again as necessary.
  • Determines if case is assigned to intake or continuing.
  • Contacts JPD clerical if additional documents or information are needed.
  • Receives SCD 692 “Federal Medicaid (Title IV-E) Eligibility/Ineligibility and Medical/Financial Plan” from the PO.
  • Completes a “PAC Checklist” and case summary on the “Contact-Action Summary” (SCD 163), then forwards the case folder to the PAC CST for Application Registration.
