Severely Emotionally Disabled (SED)
Applications
The PAC EW receives the SED application “AB3632/882 SED Out-of Home Placement Payment Authorization”+ from Mental Health, and takes the following actions:
- Reviews the application to see that it’s complete and checks the Resource Data Bank to determine if the provider is a known eligible facility.
- Contacts the MH Case Manager for missing information and/or checks CalSAWS and CDS Archive to get needed information.
- Contacts the Management Analyst to verify provider and approved rate when needed (primarily for out-of-state placements).
- Contacts the MH Case Manager if the provider is not an eligible facility or the rate requested exceeds the allowable rate.
- Completes case summary on the “Contact-Action Summary” (SCD 163) and PAC Checklist.
- Forwards the application folder to the PAC CST for Application Registration.
Continuing Cases
Mental Health uses the same application form to update the EW about the child’s SED status. For example: the SED contract has been extended; the child has left the placement; the child is in a new placement; and/or, SED is to be terminated.
The PAC EW receives all of the SED updates from MH, and must ID each one to see if it is a continuing SED case. The application update is then forwarded to the assigned Continuing EW for processing.
