Resource Family Approval (RFA)
RFA is required when a Foster Care child is placed with a relative. AFDC-FC funding may not be authorized until the home approval certificate has been received. Refer to Resource Family Approval Program (RFA).
The following forms are completed by the SW/PO. The SOC 158A must be included with the “Resource Family Approval Certificate” form (RFA 05A) for RFA homes. Copies of the required documents must be imaged in the eligibility file.
- RFA 05A - “Resource Family Approval Certificate” To be used at the initial placement
- RFA-06 - “RFA Approval Update” To be used at the annual reassessment
