General

The FC EW must review each form required and complete the “County Use Only” sections.

Child Support Good Cause Claim for Noncooperation (CW 51)

A completed CW 51 by the SW/PO it may be received for one or all absent parents. One CW 51 for each absent parent, including absent parents of infants receiving infant supplements. The FC EW must determine if the CW 51 is in imagingIDM.

When a new CW 51 is received during the RD process the Absent Parent page in CalSAWS for the named absent parent must be completed.

Note: The FC EW must complete the Collect Absent Parent Information Window as outlined in the Common Place Hand Book “Child and Medical Support Activities,” page 31-1].

Reminder: The CW 51 can only be completed by the SW/PO.

Statement of Facts Support Eligibility for AFDC Foster Care (FC 2)

The FC EW shall review the FC 2 for completeness and changes.

When the FC 2 is incomplete, the FC EW must notify the SW/PO or LG immediately to complete the document before the RD due date.

Any changes in the circumstances must be immediately acted upon by the FC EW. The FC EW must complete the “Eligibly Worker Only” section and sign and date the FC 2.

Statement of Facts Supporting Eligibility for AFDC-Extended Foster Care (FC 2NM)

The FC EW must review the FC 2 NM for completeness and changes.

When the FC 2 NM is incomplete, the FC EW shall notify the SW/PO or LG immediately to obtain the completed document before the RD due date.
Any changes in the circumstances must be immediately acted upon by the FC EW.

The FC EW must complete the “Eligibly Worker Only” section and sign and date the FC 2 NM.

Determination of Federal AFDC-FC Eligibility (FC 3)

The FC EW must review the FC 3 in the imaged case for accuracy.

A new FC 3 is not necessary after the initial determination of federal or non-federal eligibility.

Supervised Independent Living Placement Approval and Placement Agreement (SOC 157A) and SILP Inspection Check List (SOC 157B)

The SOC 157A and SOC 157B must be completed annually when the NMD is placed in a SILP. The SOC 157A/B must be signed and dated by the SW/PO and the NMD. When there is a designated payee that is not the NMD the designated payee must also sign the SOC 157A.

Six-Month Certification of Extended Foster Care Participation (SOC 161)

The SOC 161 is the form that provides the FC EW with the necessary information for continued payment. It must be updated every six months. Every attempt should be made to align the dates with the eligibility RD due date. The SOC 161 must be signed and dated by the SW/PO, NMD and FC EW.

Foster Child’s Data Record and AFDC-FC Certification (SOC 158A)

These sections must be completed on the SOC 158A:

Preplacement Preventive Services

The agency responsible for placement and care of the child shall provide preplacement preventive services to children placed into foster care on or after October 1, 1983. In addition, the service (SW/PO) case must have documentation stating why provision of these services was not successful in maintaining the child with his/her family.

Assessment and Service Plan

The placement worker must develop a written assessment and service plan within 30 days from the date the agency became involved with the child or the date the child was removed from his/her home, whichever is later.

The assessment must be updated at the periodic review, or in a nonrelated guardianship case, every six (6) months.

Periodic Six-Month Court Review

Periodic reviews shall be conducted on behalf of the child no less frequently than once every six months.

Exception: Court reviews are not required when dependency has been dismissed for a child living with his/her nonrelated legal guardian.

When no six month review date has been entered, the FC EW must contact the SW/PO:

  • To determine the status of the six month review, AND
  • To request that the SW/PO write a memo or e-mail to document that the six month review has been initiated in a timely manner but, the review process was continued by the Court.

Note: For intake, when the period between detention and disposition of a case is longer than six months, the case is out of federal compliance and therefore INELIGIBLE for foster care or services.

SOC 158A - E line, #2 and 3

Family Reunification Services

The placement worker must provide family reunification services, unless there has been a Juvenile Court finding that such services are inappropriate. In such a case the Court would order that the child receive permanent placement services.

Permanency Planning Hearings/.26 Hearings

PPH/.26 Hearings shall be conducted on behalf of the child within 12 months of the date of placement into foster care. PPH shall also occur no less frequently than once every 12 months following the first hearing throughout the period of foster care placement, with the following exceptions. PPH are not required for a child:

  • Living with his/her nonrelated legal guardian, or
  • Who is freed for adoption and placed in the adoptive home identified in the previous PPH, pending finalization of the adoption.

First Permanency Hearing Findings

FC EWs must verify that in the court order the court made a finding that the agency has made “reasonable efforts to finalize the permanency plan” within 12 months of the “date the child entered foster care”.

If this finding is not made timely, the child ceases to be eligible for Federal AFDC-FC at the end of the 12th month after he or she entered care and remains ineligible until the finding is made.

Ongoing Permanency Hearing Finding

FC EWs must verify that in the court order the court continues to make a finding that the agency has made “reasonable efforts to finalize the permanency plan” every 12 months from the date of the original permanency planning hearing.

Child Visits

The placement worker must visit the child at least once every six months.
