Rates for Specialized Adolescent Care Homes (ACH)
Effective July 1, 2018, the ACH rate was discontinued by the DFCS. The information contained below is for historical purposes only.
Certain RFs which demonstrate ability and interest in working with adolescents will receive the ACH rate. The home must be licensed and approved through the county to receive this rate. The RF may begin to receive the higher rate upon completion of training. The adolescent care rate applies to children from 12 to 17 years of age.
The rate for ALL specialized adolescent care home placements is $932 per month, effective 1/1/08.
In order to make the payment, the EW must first verify the home appears on the “Santa Clara County Licensed Adolescent Care Homes” roster found in the EW Tools folder on the shared drive [S:drive>Sharelib-DFCS>EW Tools>Licensed Adolescent Care Homes].
DFCS will retain records confirming RFs appearing on this roster meet all the criteria necessary to receive the higher rate. This includes the following:
- Verification that the home has completed the required training.
- The child is in the required age range.
- The child is in Santa Clara County (this rate does not apply to children placed outside the county).
If the RF does not appear on the listing but the rate is being requested, e-mail Social Work Supervisor, Out of Home Care Unit, with the provider name to verify eligibility.
Documents Required for Payment
The SOC 158A is required for payment. The SW indicates ACH on the rationale line of the form. This payment is not eligible for the annual CNI.
Note: KG and AAP are not eligible for this rate.
