Transitional Housing Placement Program (THPP)

THPP is a county optional program that is intended to prepare youth who are aging out of the foster care system and who are participants in the ILP to assume responsibility for meeting their own needs by providing supervised apartment living services. The purpose of the program is to increase the youth's level of self-sufficiency to enable their successful transition from the child welfare system.
Participation Requirements
An eligible participant must:
- Be 16 to 17 years of age
- Have dependency or wardship established,
- Be in an out-of-home placement, under the supervision of the DFCS or JPD, and
- Participate in the ILP.
Note: NMDs may participate in THPP but can only receive the THP-NMD rate and the difference must be paid through county-only funding.
Reminder: Children who are placed in out-of-home care solely as the result of an IEP are not eligible for THPP unless they are in foster care, either as a dependent or as a ward of the court.
Additionally, THPP participants who have children are eligible for THPP provided they meet all the above eligibility requirements. The infant supplement is available for parenting teens at the foster family home infant supplement rate.
Placement Requirements
A THPP placement provider must be a state licensed community care facility and must be certified by the County for meeting program requirements for THPP.
A provider may be both licensed as a foster family home (or certified as a foster family home by an FFA) as well as certified by the County as a THPP provider as long as they meet the requirements of both programs.
Rates
Payment rates for THPP are set by the individual county and approved by the state. Refer to chapter 3 of the Charts Handbook for current rate information. The THPP providers in Santa Clara County are:
- Bill Wilson Center
- First Place for Youth
- St. Andrew's Residential (S.T.A.R.) or Star House
Documentation Required for Payment
A SOC 158A with the correct placement information is required to make a THPP payment. The THPP payment is issued in CalSAWS in the same manner as all other provider payments.
