Register Applicant

  1. Triage
    1. Completes the SCD 2321 and attaches to the request for Application Registration (app reg).
      1. Note: Cases which have had a previous episode in the last 12 months should be marked as “Restoration” on the upper right hand corner of the SCD 2321. All inter-county transfers must be marked as “ICT” on the upper right hand corner of the SCD 2321.

    2. Completes program “Foster Care Checklist” (SCD 18).
    3. Refers to designated FC Clerical for app reg.
  2. Designated FC Clerical
    1. NOTE: Be sure to follow policy for AAP cases.

    2. Reviews documents from Triage/Intake FC EW.
    3. Completes Application Registration using the following beginning date:
      1. For AAP, the date of the AAP 2,
      2. For Kin-GAP, Intake/Triage must provide,
      3. For FC, the date the SOC 158A is signed or the beginning date of payment if no
      4. SOC 158A is provided,
      5. For Voluntary Placements, the date the SOC 155 is signed,
      6. For Wraparound, the date of the RISC,
      7. For ARC, the date the ARC 1 is signed.
        1. Note: For any additional cases the application date is the date the first payment begins.

    4. Selects add “Collect Applicant Information” from the CalWIN Application Registration Subsystem and adds the required information on the Application Window and the Address Tab.
    5. Adds the Preferred Spoken Language to the Collect Demographic Detail window when noted by Triage.
    6. Uses 373 W. Julian Street 2nd floor when no address is available for RAIC cases.
    7. Enters application information in the Collect Application Information windows and saves.
    8. Writes down application number generated by CalSAWS.
    9. Forwards to the next window queue.
    10. Adds requested program(s) (FC/AAP/KG/WRAP/CC/MC/CW) in Program Tab. Saves and moves to the next window.
    11. Collects Household Member information for the child/youth.
    12. Clears child/youth and searches for CWIN and CIN using Clear Individual through CalWIN and Clear Individual through SCI and MEDS.
    13. Assigns CIN and CWIN number as appropriate.
      1. NOTE: For AAP cases, uses the same CIN and SSN from the closed Foster Care case (if available).

    14. Repeats the above steps for additional members in the case. Saves and moves to the next window.
    15. Selects “Signed” on the Status tab and verifies the date is the same as the application date. Saves and selects the Summary tab.
    16. Selects Case Clearance, then selects “New Case #” and CalWIN will generate a new Case Number.
      1.  When there is an existing case (with the same Program), Selects the existing case number.
    17. Selects Case Assignment and assigns case based on FCE policy.
