SSN Card or Birth Certificate
Applying and receiving a Social Security Card.
- FC EW
- Completes “Application for a Social Security Card” form SS5, but does not sign.
- Attaches Court Petition.
- If No Court Petition Voluntary Placement, Then Attach “Voluntary Placement Agreement-Placement Request” form SOC 155.
- If No Court Petition Legal Guardian, then attach Original Birth Certificate and Letter of Guardianship.
- Forwards packet to SSN Application Basket.
- Designated FC Clerical
- Receives request.
- Signs the SS5.
- Mails the packet to Social Security Administration including the following:
- Completed SS5,
- Copy of Designated FC Clerical’s badge,
- Court Petition or SOC 155 or Letter of Guardianship,
- Temporary Medi-Cal Card, and
- Original Birth Certificate.
- When SSN Card is received by FCE:
- Completes ID.
- Makes a copy of the SSN Card.
- IF FCE request, then
- Adds CalWIN case number to the copied document.
- Creates a TMT and forwards copy to assigned FC EW.
- If not FCE request,
- Forwards copy to assigned Social Worker
- IF FCE request, then
Applying for a Birth Certificate
- FC EW
- Completes request “Verification of Birth, Death, Marriage, Divorce” form SCD 109.
- Forwards SCD 109 to Birth Certificate basket.
- Designated FC Clerical
- Receives request,
- Completes “Application for Verification of Birth, Death, Marriage, Divorce” form SCD 58 including the date and initials,
- Completes and forwards request to Accounts Payable.
- Forwards a copy of the completed SOC 109 to requesting FC EW,
- Files SCD 109 in “Birth Certificate Binder.”
- When Birth Certificate is received:
- Completes ID,
- Copies the original birth certificate,
- Forwards the original and a copy of the birth certificate to the assigned FC EW.
- If the request is for out-of-state, follow the policy outline in the Common Place Handbook 21.5.

Note: The original birth certificate is needed by the FC EW to apply for a SSN Card.
