4811 - SSN Match Attempt Identified Probable Transcription Error - Urgent
This daily alert is generated when the SSN failed to verify for this individual based on a match to SSA's NUMIDENT file, which is the master file for SSN issuance. SSA identified a probable transcription error as the reason for the match failure.
This alert message may be issued in conjunction with message 4806, 4807, 4808 or 4809. Other messages may be generated as well.
Action Required
- The EW must check CalSAWS, MEDS and case records to determine:
- If the SSN was correctly reported to MEDS, and
- If there are any SSN Verification discrepancy messages that may accompany this alert (e.g., 4806, 4807, etc.).
- If the SSN was not correctly reported to MEDS, enter the correct SSN on the SSN Detail page.
- If there are other discrepancy messages, refer to those messages and follow the steps to have the discrepancies resolved.
- If necessary, refer the client to SSA with form MC 194 to resolve any SSA data discrepancies and/or obtain a corrected Social Security card.
