Case Narration - General Guidelines

When working on a case narration, it is important to consider the following  guidelines:

  • Keep in mind who the reading audience will be:
    • VS Employment Counselors
    • VS Officer of the Day (OD)
    • Appeals Officers (AO)s
    • Hearing Officers
    • Eligibility Staff
    • VS Participant. 

Reminder: Clients/participants have the right to review their case record, including documentation.

  • Use factual documentation. The case narration should never include a staff member's feelings about or criticism on the participant or another staff member.
  • Ensure narrative is accurate and it does not conflict with other narrative entries or other information in the case. Provide an explanation/clarification if conflicting information is entered. 
  • Make every effort to document all case activity immediately after it occurs to avoid overlooking important information. 

Case Narration Tool

The Case Narration Tool (CNT) must be used to document the following:

  • Orientation
  • Intake
  • Orientation and Intake Assessment
  • Reassessment

The CNT may also be used to document participants' contacts. 

The narrative in a VS case must include all items included in the CNT, as applicable. However, any additional information not itemized in the CNT must also be included and can be added by using the "Additional Journal" field.
