Vocational Services - Case Narration
Purpose of Thorough Case Narration
The case narration is a written record which fully documents an individual's participation in the Vocational Services (VS) program.
Thorough case narration enables county staff and outside authorized individuals to be efficient in reviewing and understanding the case record.
Case narration should include clear details that tell the story of the individual's participation and of the actions taken on the case.
Important information to be addressed in the case narrative is, but not limited to:
- Activities the participant is assigned to, based on the individual assessment.
- Referrals the participant is provided.
- Services offered to the participant.
- Transportation /ancillary payments issued.
- Receipt of attendance and progress reports.
- Required forms provided to the participant, or received from the participant.
- Telephone calls or contacts with the participant, including the reason of the contact and the outcome.
- Any pertinent information that can help the reviewer in understanding the course of action.
Thorough case narration not only assists in telling the story, but also serves as a source for other agency staff to identify information which can assist with program's eligibility determination, county hearings, incoming OD phone calls, as well as assisting staff to provide appropriate and timely customer service. Poor or lack of documentation may lead to case errors.
