Intake Packets

Applicants for public assistance are required to complete applications mandated by the State. The applications may vary based on the program or programs for which they are applying. The application and other County forms provided to applicants for their completion are assembled and referred to as Intake Packets.

Intake Informational Notices

Informational Notices contain informational materials which must be provided to the clients when an application is received.

The following are links to the required forms in the Intake Informational Notices for CalFresh, CAPI, CalWORKs, General Assistance and Medi-Cal and may be shared with the clients via email.

 

Important: When providing the above links via email to clients, be sure not to include any Personal Identifiable Information. Refer to DEBS-BP Handbook Chapter 14, Confidentiality & Personal Identifiable Information (PII).

Clerical Preparation

Intake Packets and Intake Informational Packets are prepared by clerical staff and include the mandatory forms which must be completed or provided for each program. The forms for both of these types of packets are located in DEBS Forms webpage.
