Redetermination Forms
The redetermination (RD) forms to be completed include, but are not limited to:
- Application for CalFresh, Cash Aid, and/or Medi-Cal/Health Care Programs (SAWS 2 Plus)/Statement of Facts (SOF)
- When both parents are in the home, the SAWS 2 Plus/SOF must be reviewed with both, and both parents must sign and date the SAWS 2 Plus/SOF.
- All questions on the SAWS 2 Plus/SOF must be answered, and the answers must accurately reflect the client’s circumstances. Refer to Chewable Byte 25-120: How to Generate SAWS 2 Plus Form with the Correct Case Data for CalSAWS instructions.

Note: If a valid application is on file at intake or RD for a CalWORKs/CalFresh combo case, and the SAWS 2 Plus/SOF is signed to meet CalWORKs requirements (such as due to incomplete or incorrect information on the application being corrected), the presence of an additional signature in a combo case is not considered a duplicate signature for CalFresh. Although CalFresh is a single‑signature program, an additional signature collected in a combo case is treated solely as a CalWORKs requirement. The reason for the additional signature must be clearly documented that the second signature is required only for CalWORKs, not for CalFresh.
- Rights, Responsibilities and Other Important Information (SAWS 2A SAR)
- When both parents are in the home, the SAWS 2A SAR must be reviewed with both, and both parents must sign and date the SAWS 2A SAR.
- The Social Services Benefits Specialist (SSBS) must complete the Eligibility Worker section of the SAWS 2A SAR since it serves as county certification that the client has been informed of their rights and responsibilities. Refer to the SAWS 2A SAR Eligibility Worker Section in Application Forms, which also applies to RDs.
- Would You Like to Register to Vote? (SCD 508). Refer to Common Place Handbook Chapter 25 Voter Registration for business processes and additional information.

Important: Benefits must not be withheld or delayed because of voter registration requirements. Public assistance benefits are not affected by the applicant's decision to register or not to register to vote.

Depending on each case's specific situation, additional forms listed in Case Record Forms may be required in the case record.
Renewal Packets
CalSAWS mails out the Renewal Packet via a batch job on the 15th day of the month prior to the month the RD is due. If the case is discontinued prior to the batch job, and the SSBS later rescinds the case, then the SSBS needs to manually:
- Print the appropriate Renewal Packet in the CalSAWS Template Repository, and
- Send out all of the SCD forms listed in the Renewal Packets on the DEBS Forms Library.
Note: The Renewal Packet mailed out via a batch job includes the SCD forms. The Renewal Packet in the Template Repository is generic for all counties to use and does not include the SCD forms.
