Service Requirements for Social Workers and Probation Officers

The SW/PO must cooperate in the RD process by:

  • Completing the FC 2.
  • Completing the FC 2NM when the NMD is unavailable or unable.
  • Providing a current SOC 157A/B for NMD’s SILP Placements.
  • Providing a current and complete SOC 158A.
  • For NMDs, providing a current and completed SOC 161.
  • When applicable, providing a current home approval or license.
  • Completing the CW 51 when necessary.

Non-Dependent Legal Guardians

Non-Dependent LGs established by juvenile court or probate court are required to complete the RD documents. When documents are not returned their benefits may be terminated.

The SW/PO does not complete the SOC 158A for non-dependent children receiving non-federal AFDC-FC funding.

RD Documents Not Returned by SW/PO/Legal Guardian

In order for the RD to be complete, all documents must be completed and signed by the SW/PO or LG. When the RD document are not returned by the due date, the FC EW must initiated the FC Escalation Process outlined in FC Update 14-09.

When the youth is receiving extended LG benefits after the age of 18 years old and the LG does not complete the RD documents, benefits may be terminated. In order to restore benefits, the LG must apply to re-enter the extended non-minor former dependent program.
