ICAMA (MC only) Transferring to Another California County from Santa Clara County
The EW must initiate an Intercounty Transfer (ICT) when notification is received that an existing AAP or Kin-Gap ICAMA case has moved to another California county.

1.a | FC EW | The Continuing EW receives a change of address information from AAP parent(s) or Title IV-E KG guardian(s), the sending state, or the new county of residence. | Follows AAP Change of Address Within California process or Kin-GAP Change of Address (SCD 2339). |
1.b | Foster Care Coordinator | Receives an update request (form 7.00 for System cases or 7.5 for paper-based cases). | Sends FC Clerical the form and requests that a task be sent to the EW. |
1.c | FC Clerical | Receives the update request (form 7.00 for System cases or 7.05 for paper-based cases). (Youth has an active Medi-Cal case but the move is to another California county.) |
