ICAMA (MC only) Transferring to Another California County from Santa Clara County

The EW must initiate an Intercounty Transfer (ICT) when notification is received that an existing AAP or Kin-Gap ICAMA case has moved to another California county. 

 

Stage Who  Request Action 
1.a FC EW The Continuing EW receives a change of address information from AAP parent(s) or Title IV-E KG guardian(s), the sending state, or the new county of residence. Follows AAP Change of Address Within California process  or Kin-GAP Change of Address (SCD 2339).
1.b  Foster Care Coordinator  Receives an update request (form 7.00 for System cases or 7.5 for paper-based cases).  Sends FC Clerical the form and requests that a task be sent to the EW.
1.c FC Clerical Receives the update request (form 7.00 for System cases or 7.05 for paper-based cases).
(Youth has an active Medi-Cal case but the move is to another California county.)
  • Completes identification.
  • Sends the form and task to the EW (copies FC Coordinator).

  • Completes the ICAMA 7.00 form in the System or completes the ICAMA 7.5 for the paper-based case.
  • Completes the MC 360.
  • Completes the MC 360R.
  • Prints the MC 250 from Imaging and includes in the ICT packet.

 

 

 

 

 

 

 

 

  • Sends the 7.00 or the 7.5 to the sending state.
  • Sends copies of all documents to CDSS ICAMA Administrators.
  • Sends copies of all documents except the MC 360 to the adoptive parent(s) or legal guardian(s).
  • Sends copies of all documents to the receiving county. Refer to ICAMA Required Forms for a list of documents.        
  • Images all documents to the case file.
  • Sets case alert for 30-day follow-up to verify status in MEDS and confirms the receiving county has activated Medi-Cal.

 

 

 

3. Receiving County
  • Receives documents.
  • Forwards the MC 360R to EW.

FC Clerical

 

 

  • Receives the MC 360R and/or any form of acknowledgment from the receiving county.
  • Completes identification.

Forwards document to assigned EW.

 

5. 

 

 

 

 

 

 

 

 

 

 

FC EW

 

 

 

 

 

 

 

 

 

 

  • Receives MC 360R and or any form of acknowledgment from the receiving county.
  • Images documents for case file.

 

 

 

 

 

 

  • Completes the necessary CalSAWS entries to close the case.
  • Prints and sends NA 791 or the NA 403A to the initiating state.
  • Forwards closed case to EWS.

NOTE: MC cannot be discontinued until the receiving county acknowledges receipt of the ICT and is ready to activate MC.

6.  EW Supervisor Reviews documents.
  • Validates closure.
  • If incorrect returns to FC EW.
