IN95 - Federal/State MEC Inquiry Request
- Allows the MEDS User to search for the mail history of an individual’s Form 1095-B by entering MEDS ID (SSN) or CIN (Client Index Number), MCAP-ID or CCHIP-ID. MEDS Users also has the ability to limit the search to a particular tax year or all stored tax years.
- Refer to “ALIEN INDICATOR EXCLUSION TABLE” below if there is no 1095-B form generated for the selected tax year.
To Obtain
- From the blank MEDS Screenblank MEDS Screen, type IN95 and press Enter. The Federal/State Inquiry Request screen is displayed.
- EnterEnter MEDS ID (SSN), CIN, MCAP-ID or CCHIP-ID in the CLIENT-ID field on IN95 screen and press Enter. The IN9S, Federal/State MEC Summary screen will appear.

ALIEN INDICATOR EXCLUSION TABLE
- The excluded CITIZEN/ALIEN-IND codes listed on the table below indicate the Form 1095-B not generated. The indicator can be found on INQE screen.
- The Form 1095-B will be generated if the CITIZEN/ALIEN-IND code is changed to the acceptable code.

CORRECTION RECORDS
- In some situationsIn some situations, an update to MEDS record, not in the IN95 (change in MEC eligibility, citizenship status, or SSN) results in a correction to the Form 1095-B record. If a subsequent update in MEDS is made while the Form 1095-B correction is pending, the first correction record will be cancelled. The original correction record will display as cancelled and the new correction record will display as pending (blank PROCESS DATE) until the record is sent for processing, at which time the PROCESS DATE will be updated. The example below illustrates the results in the IN9S screen.
- If a MEDS record with a previously processed original Form 1095-B has a correction to MEC coverage and there is an outstanding T (Tax filer) reprint record, the correction record cancels the outstanding T reprint process. - If a MEDS record with an original Form 1095-B pending for processing has a correction to MEC coverage while there is an outstanding O (Original) reprint record, the correction record cancels the outstanding "O" reprint process.
