CalWORKs Home Visiting - Eligibility Criteria Partnership Models
There are three (3) home visitation models that bring deep experiences in the area of home visiting that will provide a coordinated and integrated approach to service delivery to children from prenatal up to age 48 months utilizing cultural competency. Although each home visitation model serves a specified population, the home visiting programs will collaborate to provide service integration to ensure continuity of service for all eligible CalWORKs families, including CWES participants. If the custodial parent chooses to include the non-custodial parent in participation, they can notify the HVP provider who will coordinate services at the discretion of the custodial parent.
Nurse Family Partnership (NFP)
NFP, which is administered through the PhD, provides 24 months of home visiting services for mothers who enroll in their first or second trimester of pregnancy. To qualify, the client must be at 28 weeks gestation or less. This program serves mothers who are at 28 weeks gestation or less, with no other children in the home.
Strong Mom Strong Babies (SMSB)
SMSB, which is also administered by the PHD, serves an expanded population of parents with multiple children, first time mothers enrolling later than their second trimester, caretaker relatives of children less than 24 months of age, and child-only AUs. Services are provided until the children reach 24 months of age.
Parent Child Home Visitation Program (PCHVP)
In partnership with PHD, the PCHVP is administered by First 5 of Santa Clara County, and is designed to augment NFP and SMSB programs by extending home visiting services to families with children up to 4 years (48 months) of age. At age 4, children and their families who still want to continue with home visiting services can continue through the Kid Connections Network of Providers and/or the First 5 Public Health Nursing Home Visiting Program.
Note: To fully benefit from program services, a child should be between 25 and 35 months of age at enrollment.
Public Health Department (PHD) Referral Center
To facilitate service coordination, the home visiting referral center will forward referrals to the appropriate service provider who will coordinate service delivery with the assigned EC.
Service providers have strength-based practices for working with families with unmet needs and a general understanding of client access to CalWORKs; Medi-Cal; CalFresh; WIC programs; and other resources.
