CalWORKs Home Visiting Outreach and Application
Outreach
Participation in the HVP is voluntary and outreach includes but is not limited to designated mailers by service providers, “CalWORKs Home Visiting Initiative” (SCD 2552) outreach flyer, email, social media and referrals. Eligible families are referred by:
- EWs for families:
- With a WTW requirement, or
- Without a WTW requirement (examples: non-needy caretaker relatives, Ineligible non-citizens, Safety Net/Timed-Out)
- ECs for families with a WTW requirement who are WTW mandatory or voluntary
Application
Participation in home visiting is voluntary and client may voluntarily terminate home visiting services at any time. To participate, client may complete the “Home Visitation Initiative Informing and Opt-In” form (CW 2224) indicating desire to opt-in to volunteer. A copy of the CW 2224 can be provided to client. The client is informed that a referral will be initiated, and that a HVP service provider will be contacting them.
If client chooses not to volunteer in HVP, a copy of the CW 2224 is sent to IDM for documentation.
CalWIN case comment type, “CalWORKs Home Visiting,” is used to document services offered and participation outcome.
Note: If client chooses not to opt-in, client may choose to opt in at a different time, if still eligible.
