4820 - Surname Discrepancies Identified - Action

This message may be issued along with message 4801, 4802, 4803, 4804, 4805, 4806, 4807, 4808 or 4809. It indicates that the surname submitted on the SSN Verification Request did not match the surname found for that SSN on Title XVI (SSI/SSP) and/or Title II (RSDI) data.

Action Required

  • The EW must check CalSAWS, MEDS and case records to determine:
    • If the client’s surname was correctly reported to MEDS.
    • If there are any SSN Verification discrepancy messages that may accompany this alert (e.g., 4801, 4802, etc).
    • If two different individuals have been inadvertently linked to the same SSN on MEDS.
  • If the surname was not correctly reported to MEDS, enter the correct surname on the Individual Demographics Detail page.
  • If the client’s Social Security card has a different name, enter the SSN name as an alias in the Previous Names page section of the Individual Demographics Detail page.
  • If there are other discrepancy messages, refer to those messages and follow the steps to have the discrepancies resolved.
  • If the MEDS record has the same SSN for more than one individual, a referral must be made via a GadWIN ticket to the MEDS Coordinator for assistance in getting the problem corrected.
  • If necessary, refer the client to SSA with form MC 194 to resolve any SSA data discrepancies.
