Cal-Learn Program - CalWORKs Home Visiting with Cal-Learn Participants
Cal-Learn participants may volunteer in the CalWORKs Home Visiting Initiative (CWHVI) Program. Refer to the CalWORKs Home Visiting Program for additional information.
Services Offered
During the course of case management, PPMM explains the home visiting program to Cal-Learn participants. When the client accepts home visiting services, PPMM staff notifies the assigned Cal-Learn Associate Employment Counselor (AEC)/Employment Counselor (EC) that the client chooses to volunteer in CWHVI.
Referral
Upon receipt of a referral from PPMM, the Cal-Learn AEC/EC makes the CWHVI referral using the DEBS Referral Application. The Cal-Learn AEC/EC needs to include within the Comments section the assigned PPMM case manager’s name, email address, and phone number. Refer to Referral Process for referral steps.
Status Reports
Monthly status reports are distributed as follows:
- Santa Clara County Public Health Department (PHD) staff forwards status reports via email to CWES at: ssa_cwescm_fax@ssa.sccgov.org
- CalWORKs Employment Services (CWES) clerical staff distributes status reports via email to the assigned Cal-Learn AEC/EC
- Cal-Learn AEC/EC forwards status reports via email to the PPMM staff assigned to the Cal-Learn case.
