Change of Address for Foster Care Children Placed Out-of-County

When a FC or AAP child has moved to a new county of residence, he/she must be disenrolled from the COHS in the former county within two working days upon notification of the address change.

  • The original county where the court has jurisdiction retains responsibility for the child. It is the RESIDENCE ADDRESSS and RESIDENCE COUNTY CODE on MEDS that determines which managed care plan is available to the child.
  • Although Santa Clara is not a COHS county, the residence address and Residence-County code for a FC and AAP child, including those who also receive SSI benefits, must be updated IMMEDIATELY in CalSAWS and MEDS. This is to ensure the child can quickly obtain medical and mental health services in their new county of residence through fee-for-service providers or through a Medi-Cal managed care plan; especially if the county they move to is a COHS county.

Residence Address / Residence County Code

The address of where the child is placed must be entered in CalSAWS and MEDS. The following steps apply when updating the out-of-county address for a FC or AAP child:

The EWs will:

  • Enter the address of the facility where the child is placed in the Address Detail page.
  • Check MEDS in two working days to ensure the new address and Residence-County code are properly recorded. If necessary, submit a SCD 1296 to request an EW12 online transaction to update the address and Residency-County code.

SSI Foster Care/AAP Children

Update the Out-of-County Address and Residence County Code for a SSI Child

Additional steps are required when updating the out-of-county address and Residence-County code for a FC or AAP child who is also an SSI client.

  1. The EWs will:
    1. Enter the address of the facility where the child is placed in the Address Detail page.
    2. Submit an SCD 1296 requesting the MTO to complete an EW55 (SSI/SSP Modify) transaction to update the child's residence address AND Residence-County code on the INQM screen with the current month as the VALID-MMYY.
  2. The SSI Coordinators will:
    1. Fax/Email the designated SSA staff with a SSP-22 form to request urgent address change. (SSA system sends TRANS CODE “SD20”TRANS CODE “SD20” to MEDS on INQQ screen as an indicator for change of address.)


    2. Check if TRANS CODE “SD20” displays on INQQ screen by the fifth (5th) day.
      1. If not displayed, send an email to the SSA staff and send another email if TRANS CODE “SD20” does not show by third (3rd) day. Continue to follow up with SSA until the address is updated in their system.
      2. If displayed, ensure COUNTY code changes to “43” on INQM screen.

Correct County Responsibility Code for SSI FC/AAP Child

The following instructions are to correct the County of Responsibility code for an SSI FC/AAP child moving to Santa Clara county:

  1. The SSI Coordinators will:
    1. Notify the assigned EW or Supervisor of the day for unassigned cases to create a GadWIN ticket when TRANS CODE “SD20” exists on INQQ screenINQQ screen and COUNTY code does not show "43."


  2. The EWs will:
    1. Submit an urgent GadWIN ticket.

The ticket must indicate that the SSI child is a mental health and SSI/FC patient.

  1. The MEDS Coordinator will:
    1. Submit an urgent Remedy ticket to State to request county code change to “43.”
