4826 - 1095-B - Probable SSN/Name Mismatch Determined by IRS Name - Action
This alert is generated when an individual’s SSN/name is mismatched between MEDS and IRS data. As the result, NFRA is sent to the individual to request for correcting their SSN and/or name in MEDS or IRS file.
Note: Correction made on IN95 MEDS screen will not be updated in “regular” MEDS screen (i.e. INQM).
Action Required
- Verify a reason for NFRA mailing via IN9D - IRS 1095B Detail/Reprint screen.
- If a MC beneficiary has used a different name/SSN on their tax record than the one in MEDS (i.e. change of last name when married, having more than one SSN assigned, etc.), determine what system has correct SSN/Name and take appropriate actions accordingly.
- If IRS has correct information, follow the instructions on MEDS/CDB Record Changes for updating MEDS record.
- If MEDS has correct information, advise beneficiaries to update their tax files with correct name/SSN. An EW may request name correction (Reprint Type ‘T’) on [IN9D] screen for 1095-B reprint with the correct name.
