Health Alliance (HA) Termination of Services

When the client completes the services, or discontinues services prior to completion, the HA provider will notify the EC within five (5) working days, by emailing the SCD 29 if the HA provider is part of the WTW Plan.

Upon receipt of the SCD 29 indicating the client is no longer participating in HA, the EC must review the client’s file and determine if the client may proceed with their CWES activities under:

  • Their current WTW Plan,
  • A modified WTW plan, or
  • Requires an assessment or reassessment by an EC.
