1101 - Insufficient Information for Birth Record Match - Urgent

This alert is generated when the AP19 transaction sent to request a California Birth Record match contains insufficient information. This alert is sometimes generated with other alerts such as alert 1041.

Action Required

Check the [INQE] screen to see if the citizenship information is already recorded. If YES, no further action is required. If NO, review the [INWA] screen to determine if another alert is generated in conjunction with this alert.

If alert message 1041 is also generated, it usually means the name submitted is incomplete. A complete name consists of the individual’s last name, first name and if available, the middle name. It is also important that the names entered in the correct name fields. To resolve the alert:
- Review if client's name and birth state are entered correctly.
- Review the names (including client, mother, and father) to make sure they are correct and that they are entered in the correct fields in the Birth Certificate Information page section of the Vital Statistics Detail page (e.g., first name not entered in the middle name field).
- Reenter the names if incorrect and resubmit a Vital Statistics Birth Match.
If no other alert message(s) is generated with this alert, review the Individual Demographics Detail page to ensure ALL required data for a birth match is entered and resubmit a new request via the Vital Statistics Birth Match .
