Viewing Individual Inquiry Screens
There are two ways by which the EW may view different individual detailed inquiry screens:
- USING THE ENTER KEY
From the Individual Inquiry Summary screen, press the Enter key to display individual screens which contain a positive response. The screens are displayed in the order they are listed: WA, UI, DI, FT, T2, and 16. By continuously depressing the Enter key, the EW will scroll through all pages with positive data.
- SELECTING AN OPTION
From the Individual Inquiry Summary screen, enter a highlighted Option (e.g., WA, UI, etc.) in the SELECT OPTION field, and press the Enter key. This allows the EW to go directly to the desired screen.
EDD Wage & Employer Inquiry Screen (Option WA)
This is the main detail screenmain detail screen for information returned from EDD on their Wage File. Up to five employers are returned but only two are listed on each screen. If there are three or more employers, a message “MORE EMPLOYERS - HIT ENTER KEY TO DISPLAY” will be displayed and highlighted. If the ENTER key is depressed, the additional employer information will be displayed.
If the individual is not employed in the current quarter, but had employment in prior quarters, EDD will display the total earnings in each prior quarter. The last employer is displayed with earnings showing zero.

EDD UI Inquiry Screen (Option UI)
This is the main detail screenmain detail screen for information returned from EDD on their UI File. All the unemployment information on the database is displayed on this screen. Line 4 contains the date the information was reported and the first letter of the first and middle names and the first 8 letters of the last name.

EDD DI Inquiry Screen (Option DI)
This is the main detail screenmain detail screen for information returned from EDD on their DI File. All the DI data on the database is displayed on this screen. Line 4 contains the date the information was reported and the first letter of the first and middle names and the first 8 letters of the last name.
FTB Inquiry Screen (Option FT)
FTB provides information on up to 20 payors. IEVS Abstract will display the total number of payors and the total amount of interest and dividends. However, only detailed information for the five highest accounts will be displayed online.
If there are three or more payors, a message indicating “MORE PAYORS - HIT ENTER KEY TO DISPLAY”“MORE PAYORS - HIT ENTER KEY TO DISPLAY” will be displayed and highlighted. If the ENTER key is depressed, the additional payor information will be displayed.
The following EARNING TYPE Codes are displayed below each account number on the FTB report:
- 1 = Dividends
- 3 = 1098 (Mortgage Interest)
- 4 = 1099A (Acquisition or Abandonment of Secured Property)
- 6 = Interest
- 7 = PATR (Taxable Distributions Received from Cooperatives)
- 9 = 1099R (Distributions from Pensions, Annuities, Retirement Plans, etc.)
- A = Misc (Miscellaneous Income)
- B = 1099B (Proceeds from Broker or Barter Exchange)
- D = OID (Original Issue Discount (Interest))
- F = 1099G (Certain Government Payments)
- L = 5498 (IRA Contribution)
- S = 1099S (Proceeds from Real Estate Transactions)
- W = W2G (Certain Gambling Winnings)
Title II Inquiry Screen (Option T2)
This screen provides Social Security Title II informationSocial Security Title II information including current entitlement status and previous payment amounts. This information is current as of the date of the match. The screen also provides Medicare Part A and Part B information including Buy-In status.
If the Name and DOB of the individual on IEVS is different than the Name or DOB on the Social Security file, no information will be returned.

Title XVI Inquiry Screen (Option 16)
This option provides informationoption provides information on the client’s SSI/SSP Title XVI benefits including payment status, current and previous payment amounts. This information is current as of the date of the match.
